ST Plastic Surgery) About Nose Revision Surgery

Continued from the last week, we would like to talk more about Rhinoplasty. Since the nose is the center of the face and every individuals have different condition such as the thickness of the skin, amount of cartilage, desired look or even more, nose surgery must be seriously considered based on the detail consultation with experienced surgeon.
However! It is not always easy to meet a great doctor at first and your first surgery could go wrong therefore you might need to consider a revision surgery to fix it.
Majority problems of nose surgery are caused by inserted silicone (even silicone is one of the most safe implants used for plastic surgery) or sculpted tip.
Today let's find about silicone related problems!
Nose Revision Surgery
Caused by Nasal Implant

1. Moveable Implant
If the implant inserted area was not fixed under the periosteum but placed under the skin or the periosteum was damaged, the nasal bone and periosteum should by precisely detached so that newly inserted implant would not move after fixation.

2. Deviated Implant
If silicone implant is inserted on the deviated nasal bridge for augmentation, the nose could look more severely deviated. Revision surgery should be combined with a surgery to correct the original deviation of nasal bridge. In case of having deviation while growing up, it can be improved by Osteotomy accompanied by nasal septum surgery. In case of minor deviation of nasal bridge, it can be partially improved by cartilage and dermal fat.

3. Low Inserted Implant
Nasal bridge which seems to be lower than expected, nose revision surgery should be planned by considering the amount of extra skin. In case of enough skin on the nasal bridge, the previously inserted implant is removed and new silicone or gore-tex can be sculpted for desired height. If not, it is possible to graft autologous tissue for augmentation.

5. Transparent Implant Through the Skin
If implant such as silicone or gore-tex is forcefully used when there is not enough skin on the nasal bridge and nasal tip, it causes skin thinning and capsular contracture that makes the feature of implant visible. It also promotes the generation o capillary which makes redness on the nasal bridge and contraction of the nose. Nose revision surgery requires to include removing the inserted implant and rather using cartilage, temporal fascia, dermal fat, or artificial skin.

6. Foreign Body Such as Paraffin
Not implant, but the use of foreign liquids such as paraffin, liquid silicone, Vaseline or etc. that are not supposed to be used for Rhinoplasty, the injected foreign body reacts to tissue and permeates, and causes skin discoloration and deformity. Nose revision requires complex procedures including the simultaneous removal of injected foreign body and tissue, and use of implant or autologous tissue (temporal fascia) to solve side effects and the nasal tip lengthening.
